Enrollment
REGISTRATION  FOR  NEW  STUDENTS

Parents need to pick up the enrollment packet as soon as possible. This packet includes instructions and mandatory requirements from our district which must be met PRIOR to registering your child. Please read through entire packet immediately as some of these items may take days, weeks, or months to complete or may require an additional form.

MANDATORY REQUIREMENTS:

  • Child must be 5 years old ON or BEFORE December 2nd of 2010
  • Birth Certificate
  • Proof of Residence-current utility bill not more than 1 month old (phone bill is not acceptable) and must be under the child’s parents name. If you and your child are living with another family and the bill is not in your name you need to obtain from us the “Declaration of Residency” form which must be notarized BEFORE you register your child.
  • Immunizations must be up-to-date. You must present a yellow immunization record or equivalent of. It must have date, doctor stamp/signature, and include the following:
5 DTP shots
4 Polio shots
2 MMR
3 Hepatitis shots
1 Varicella (chicken pox shot or a physician’s documentation indicating when your child had this disease) for all incoming first, kindergarten and for students coming from
outside of California.
TB Mantoux (must NOT be more than a year old from the first day of school) TB must have been given and read with signatures and result.
  • Transfer from previous school and last report card.

All incoming 1st graders must have a Report of Health Examination for School Entry form (physical form) filled out and signed by a physician before entering in September.
